Become a Patient

Steps to follow

To become a new patient, follow these simple steps: . Complete a screening form which includes our (2) full intake packet. 3. Schedule an intake appointment. 4. Manage Ongoing Treatment.  

Step One

Complete the screening form for the service you are interested in.

You will be asked to upload pictures of your insurance cards.

TMS treatment can also be covered by insurance but requires a prior authorizations.  See screening form below that covers most of the insurance requirements/stipulations.

Step Two

Complete screening/intake document 

​

If you can attest to everything in the screening form, continue on and complete our full intake packet.

​

This includes insurance card upload, medical history, social history, family health history, current and past medications, review of systems and consent forms.

​

Once this is completed, we will call you to make an intake appointment, 

Step Three

Schedule an Intake Appointment

​

Please arrive 15 minutes early to ensure all medical history and current medications are appropriately received, and/or complete in the office.

Step Four

Ongoing Treatment

Depending on service provided we will begin the prior authorization process. For TMS it can vary as to how long this takes based on insurance.
